I got my check but also received a letter a few weeks prior to that.

The state said that my application was not in accordance with the rules but would be processed anyway.

The problem was that I did not have my name on each receipt.

What do I do if the vendor does not have me on file and thus my name does not appear on the receipt?

Can I just write my name on the receipt?

I was going to call them but anticipated a busy signal so I defer to the experts here.

A couple of seasons ago, I had a couple of receipts declined for a similar reason. My name wasn’t on the original receipt so I had written my name on it, though it was clearly in different ink and handwriting.
Speaking with the auditor reviewing my receipts, he advised that the gas receipts need to be completed in full by the supplier or they’ll be flagged. Now I check each receipt after fueling to ensure that my name, date, hull id, fuel total and cost are all completed prior to leaving the fuel dock.

The law is written this way (id the purchase) for a couple of reasons.
1. They don't want to be giving refunds to the person who fills his boat and his truck at the same gas station and tries to say it is all "off road" use.
2. Industrial companies, for example, may be filing for refunds quarterly, when they are using thousands of gallons. There is a lot more concern there about "inaccurate" reporting. When an audit is done "inaccurate" may turn into perjury. Yes, the state does do audits.

What do you do if all of the info is NOT on the printed receipt (after you've checked)?

I usually ask the attendant for a complete receipt once they’ve started fueling in an effort to head off that possibility. Depending on how busy the fuel dock is, if there’s anything missing at the end, I’ll ask them to fill it in before heading out. Most every season though, I unfortunately end up with either an incomplete one, or one I’ve misplaced.
