Need Chimney Repair recommendations

We have a chimney that is in desperate need of repair. The "above the roof line" part of the chimney needs to have its cement blocks replaced and we would love to have this capped. If you know of someone who could do the work, please let me know.

Note: We are not looking at replacing the whole Chimney at this time, just the above the roof line part needs to be repaired and capped.
